Spilling my Heart...with a letter.

I'm currently a senior in high school. There's a teacher in my school, we'll just call them "Teacher X".
In my sophomore year of HS, I came out as gay. I've gotten comments from "You don't like ME, do you? Because that's disgusting." to "You should just kill yourself. No one will care." It's a small school. I really have no other option besides dealing with it.
That same year, Teacher X came to my school. He/She took over some extra-curriculars I was a part of, and we became good friends in a sense. This year, Teacher X even requested to be my Senior Paper adviser, something that is very unusual, as the choices are almost exclusively random.
It's now college application season, and I have to find some people to write letters of recommendation for me. I've never had Teacher X as a teacher, but I still asked him/her to write an "extra" letter for me. They happily accepted.
One of the papers I'm required to include in the portfolio I'm giving to the recommenders is a "personal info" sheet, where we have to write our background, troubles, etc. I, obviously, wrote about my coming out deal. For Teacher X though, I want to include a "Thank You" letter to them. A thanks for being there for me essentially. Teacher X currently has no idea about what I've dealt with before, so I figure it'll be somewhat of a shock.
What I'm asking though is what to do. Should I write the letter? I'm not willing to speak with them, as I sort of have a speech impediment and I can organize my thoughts better on paper. I don't really know what's socially acceptable in cases like these, so I'm giving this to you guys. I want to thank Teacher X, but I don't know what to say and how they'll react.
